No eligible plan, private referral

Complete relevant forms, and referral letter with additional information.

Send all information to reception, for them to send to DDD and book initial appointment for patient HERE.

Jacinta Sherlock (they/them) is a compassionately provocative, specialised trauma-informed mental health and eating disorders dietitian who has held positions within the public mental health sector for over eight years.
Jacinta has worked across youth, adult, and forensic mental health settings. Their nutrition counselling approach is well known within the community for its capacity to identify processes impacting eating behaviour and for the use of therapeutically informed interventions to improve nutritional intake within an extended scope of practice.
Jacinta is passionate about trauma-informed dietetic practice, acceptance and commitment therapy (ACT), and collaborative leadership and unified power.
